On this webpage, you can give your server any name, it basically has no relationship with your domain name. You can select the data-center of your choice, usually there is no need to change from the default.
Scroll down to the list of available server images. For example in this guide I used Ubuntu 12.04 LTS. After selecting it, allocate the RAM you want and click the create button server; the automatically generated password will be shown as pop up window.
